Enrol with WFD
We are delighted to announce that with the arrival of Dr Janessa, and a new GP in 2024 our books are now OPEN for enrolments. Priority will be given to patients residing locally, who are without a local provider.
Enrolment forms can be downloaded here, or picked up at our reception.
Please note a separate enrolment form needs to be filled out for each person (including all children). Photo identification will need to be provided for all persons over 16 years of age.
We also have a new patient questionaire that is attached to the form - please fill ALL parts out. This helps us check that your electonic record is accurate and allows us to anticipate some of your health needs.
