Enrol with WFD
We are open for limited enrolments to residents of Waihi without a local provider. We are not accepting transfers from local medical centres.
Enrol online below, one application per person, including children. Please make sure to read our Enrolment policy, Health Information Privacy Statement, and New Patient Info.
A new patient questionnaire will be sent to all enrolments over the age of 16 - please fill ALL parts out. This helps us check that your electronic record is accurate and allows us to anticipate some of your health needs. We cannot process your enrolment form until this has been returned.
