We are open for limited enrolments.   Priority will be given to patients residing locally  who are currently  without a local provider.  We are not currently accepting transfers from local centres.

Please note a separate enrolment form needs to be filled out for each person (including all children). Photo identification will need to be provided for all persons over 16 years of age.

A new patient questionnaire is attached to enrolment the form - please fill ALL parts out.   This helps us check that your electronic record is accurate and allows us to anticipate some of your health needs.
